PANDA Membership Agreement

All members of PANDA are required to complete and submit this Membership Agreement Form. Submission of this form confirms your understanding of and agreement to abide by all membership policies and requirements.

  • Active Members are required to support and assist in the promotion of PANDA fundraisers, including but not limited to social media campaigns, raffles, live and silent auctions, Lemonade Stand Day, and PANDA Picnic.
  • Active Members are required to participate in the procurement of live auction, silent auction, and raffle items.
  • Active Members are required to assist in the set-up of the Annual Fashion Show and Luncheon as requested by the event team and attend the Friday night dinner and patient model video preview the night before the event, unless an absence request has been submitted to the PANDA Secretary.
  • Active Members are required to participate in the Annual Fashion Show and Luncheon by attending or volunteering, unless an absence request has been submitted to the PANDA Secretary.
  • Active Members are encouraged to attend the annual tour of Steele Children’s as planned by PANDA.
  • Active Members must inform the Vice President of a request for a change in membership status (going Golden) in writing by January 1. Golden status may only be requested after five (5) years of membership.
  • Active Members shall inform the Secretary and Vice President about changes to name, address, email, or telephone number.
  • Active Members agree not to copy, disseminate, or use for any non-PANDA purpose any PANDA materials, print collateral, documents, membership lists, donor lists, auction reports, or any information proprietary to PANDA, including but not limited to items available only to members in the directory and on the PANDA website.
